MySQL Proxy init

This init script was tested with MySQL Proxy and RedHat and Fedora 8

[edit] /etc/init.d/mysql-proxy

Only change PROXY_PATH=/opt/mysql-proxy/sbin var to directory where mysql-proxy bin is located.
