populating dropdown menu + php+jquery+mysql

Question

i am googled and i can't find a good tutorial that demonstrate how i can do something like this

i already check the tutorial of the author, but not satisfy me (bad tutorial in my opinion).

Preferentially using json+jquery(+ajax)+php and mysql.

If anyone know a good tutorial I'll be thankful.
